All-Inclusive Community Health Center's (AICHC) quest is to promote increased access to healthcare, social services and education, by creating linkages between community-based & faith-based organizations, businesses, educational & governmental institutions and healthcare facilities.

Our Mission

We provide comprehensive healthcare and social services to community residents, including the uninsured. Our Mission is:

Establish a Primary Care clinic in Burbank to serve the medically underserved residents in that community.
Promote the physical and mental well-being of vulnerable & special populations by improving the environmental and social conditions that adversely affect the patients and their communities.
Provide case management for special populations, including culturally competent patient education that will facilitate and promote active participation in the patient's self-management of their chronic disorders.
Conduct clinical performance measures and data analysis for patients with chronic conditions. This ensures that all patients receive timely intervention & preventive health screenings. Patients will be encouraged to be key decision-makers in the treatment process, resulting in good disease control and improved clinical outcomes.
Promoting Patient-Centric, Provider-Assisted self-management programs that ultimately result in patient self-efficacy and alteration of the course of the disease.

This is probably the nicest Community Health Center I have ever seen. It is just GREAT!
